Please try the latest revision from SourceForge, it contains a new
method ecrt_slave_config_reg_pdo_entry_pos() to register a PDO entry
using its list position instead of its PDO entry index. This method was
introduced exactly for this problem, when indices are not unique any
more. Feedback is welcome ;-)
